LISD RankOne Forms are online. Now the UIL Medical History Form is online too! Make certain these forms are completed for the 2020/2021 school year!

Know this… due to the Covid-19 pandemic, you will NOT have to get a new physical for the 2020-21 school year UNLESS your answers on the Medical History Form indicate that a new physical evaluation is necessary. Any “yes” answer to any of questions 1-6 on the Medical History Form will require a student to receive further medical evaluation, which may include a physical exam. We have been told that current athletes will be contacted by LISD if they need to get a physical exam for the 2020-21 school year based on their completion of that form.
